Position Overview:
We are seeking a dedicated and compassionate Care Coordinator to join our healthcare team. In this hybrid role, you'll focus on providing Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services to patients managing chronic conditions such as diabetes, cardiovascular disease, and COPD.
You'll play a vital role in supporting patients by offering personalized care coordination, ensuring they receive the resources, education, and support needed to thrive. Key Responsibilities
1. Patient Care Management
Serve as the primary point of contact for assigned patients, ensuring their needs are met with empathy and efficiency.
Deliver Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services in alignment with program guidelines.
Accurately document all patient interactions and care plans in the Electronic Medical Record (EMR) system.
2. Administrative Support
Meet or exceed metrics for outbound patient contacts, compliance standards, and case management.
Perform data entry and ensure all patient interactions are documented accurately in required systems.
Ensure compliance with regulatory guidelines and organizational policies.
3. Care Coordination
Assist with delivering and setting up Remote Patient Monitoring (RPM) devices as needed.
Manage pre-authorizations, referrals, prescription refill requests, and medication reconciliation.
Coordinate services with external vendors, including durable medical equipment (DME) providers.
Collaborate with Primary Care Physicians (PCPs) and other healthcare providers to deliver comprehensive patient care.
4. Patient Education & Support
Provide patient education on medications, chronic disease management, and lifestyle modifications.
Address patient inquiries, triage requests, and coordinate timely follow-ups.
Assist healthcare providers with medication adjustments and clinical concerns to optimize patient outcomes.
Qualifications
Certification as a Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), Med Tech, or equivalent designation.
Strong organizational skills with exceptional attention to detail.
Excellent communication and interpersonal skills to build trusting relationships with patients and healthcare teams.
Ability to work independently while collaborating effectively within a team.
Proficiency in using Electronic Medical Record (EMR) systems and other healthcare software applications.
