Job Description

ABOUT THE POSITION
Peer Support Services are structured and scheduled activities for individuals living with HIV. Peer
support is provided by Care Coordinator staff. Peer Support is an individualized, care-focused
service that allows individuals the opportunity to learn to manage their own health and advocacy
process. Interventions of Care Coordinators serve to enhance the development of natural supports, as
well as coping and self-management skills. Interventions of Care Coordinator staff may also provide supportive services to assist an individual in community engagement.

Peer Support Services emphasize personal safety, self-worth, confidence and growth, connection to the community, boundary setting, planning, self-advocacy, personal fulfillment and development of social supports, and effective communication skills

Examples of specific interventions include:

  1. Self-help/Self Love.
  2. Individual advocacy.
  3. Treatment adherence.

The Care Coordinator’s primary responsibilities are to direct, coordinate, and manage the activities of a comprehensive health program and to ensure the program operates in full compliance with HIPAA, relevant laws, ensure accuracy of data; compliance with regulation; and identify operational efficiencies.

  • Education and training of clients and others who have a legitimate role in addressing the needs identified in the care plan.
  • Preventive, and therapeutic interventions designed for direct individual activities.
  • Assist with skill enhancement or acquisition and support ongoing care and functional gains.
  • Develop interpersonal and community relational skills, including adaptation to home, school, work, and other natural environments.
  • Inform the client about benefits, community resources, and services.
  • Assist the client in accessing benefits and services.
  • Arrange for the client to receive benefits and services.
  • Referral linkage.

Desired Qualifications:

  • The successful candidate will possess significant experience working with vulnerable populations
    and in the nonprofit sector.
  • Prefer experience working on HIV-related issues or with people who experience HIV/AIDS. Must
    be able to work evenings and weekends a few times a month and quarterly travel.
  • Excellent verbal and written communication skills.
  • Highly compassionate and attentive to details.
  • Have a high school diploma or GED equivalent.

Physical Requirements

  • Frequent sitting and/or standing.
  • Repetitive movement of hands and fingers – typing and/or writing.
  • Visually or otherwise identify, observe, and assess.

Travel Requirements

This role does not require regular travel; however, this role may need to travel on occasion to meetings, training, or to support the organization and/or clients.
